Life Change Checklist
Employees are requested to give notification of pregnancy or intended adoption at the earliest possible time in order for the company to make the necessary organization arrangements. Employees must provide at least one month notice before the start of the Maternity/Parental/Adoption Leave, with the following information in the checklist below:
- The start date of the intended maternity, adoption or parental leave
- Part time work schedule (if applicable)
- Any vacation days planned immediately prior to your leave
- The expected date of birth/placement – indicating single or multiple births
- The intended date of return to work
- Medical certification (e.g. a doctor’s note indicating the employee’s due date) Please read the Canadian Parental Leave Policy for more information: Canada Maternity, Adoption and Parental Leave Policy
In order to add your dependent and update your extended health benefits if required, you will need to declare a “life event” using the My Benefits Portal website within 30 days of the life event. Sponsored Employees who are on a SLB Employment Visa must contact their Visa Coordinator or HR Manager, who will advise on steps to be taken to add their spouse/partner to their visa. Geomobile Employees must contact their Cartus Consultant and notify them of their dependent as well.>

Benefits Received
SLB provides new parents with either Maternity/Adoption paid leave OR Paternity paid leave. Employees eligible for the Maternity/Adoption pay will get 16 consecutive weeks of eligible pay through SLB. Employees eligible for the Paternity paid leave are entitled to 8 weeks inclusive of normal days off. Paternity leave can be taken in one continuous period or in up to four smaller increments. However, the schedule should be agreed to by the manager and employee, and the leave must be taken within one year of the child’s arrival.
